Picking Up Sacramental Certificates
In accordance with privacy policies of the Archdiocese of Toronto, sacramental certificates may only be released to:

If you are authorizing next of kin to pick it up, you can do so by writing a letter of authorization. The letter should include:
For more information or to request a certificate, please contact the Parish Office.
- Baptismal or Confirmation certificates can be made available for you upon request. Please contact the Parish Office.
- Include the full name and date of the sacrament (preferably full date; or at least month & year)
- For an ecclesiastical Marriage certificate, please contact the Parish Office.
- Include full names of both parties and date of the marriage.
- Civil Marriage & Death certificates are not issued by the parish or Archdiocese. These certificates are only issued by the Government of Ontario.
Picking Up Sacramental Certificates
In accordance with privacy policies of the Archdiocese of Toronto, sacramental certificates may only be released to:
- The individual named on the record (if 18 years of age or older)
- A parent or legal guardian (for a minor under 18)
- An authorized representative (next of kin) with written consent from the individual named on the record
Please note:
- Sacramental certificates CANNOT be sent by email.
- Certificates may be mailed to another Catholic parish, school, or institution when required for sacramental purposes.

- The name of the certificate owner, home address, and email address
The date of the letter
The name & address of the person who will pick up the certificate
Their relationship with the certificate owner (should be next of kin)
The reason for the request
The validity period of the letter
A copy of the certificate owner’s government photo ID
Government photo ID of the authorized person
